چکیده

Introduction There has been a continuous increase in the incidence of major lower limb amputations from the beginning of the century until the mid-1980s (Table 1). This increase has been attributed to the increasing number of elderly persons, diabetics and smokers (Liedberg and Persson, 1983). During the last decade, several reports from Scandinavia have mentioned a decreased incidence of major amputations (Larsson et al., 1995; Larsson and Risberg, 1988; Persson et al., 1989; Pohjolainen and Alaranta, 1988; Sernbo et al., 1996; Wahlberg et al., 1994). Others have reported a reduction in the number of major amputations (Ebskov et al., 1994; Eickhoff, 1993; Lindholt et al., 1994; Luther, 1994; Lääperi et al., 1993; Pedersen et al., 1994; Pell et al., 1994; Stenström et al., 1992). The reduction has been attributed to better foot care among diabetics (Gibbons et al., 1993; Larsson et al., 1995), and an increase and improvement in vascular surgery (Ebskov et al., 1994; Eickhoff, 1993; Giddings et al., 1993; Jeans et al.,

Controversies regarding vascular disease in the patient with diabetes: a review of the literature.

Vascular disease in the patient with diabetes represents a potentially devastating complication. Tri-neuropathy (sensory, motor, and autonomic) often predisposes patients to ulceration and vascular disease leads to delayed healing. Vascular pathology compromises blood flow and oxygen provision, affecting healing, infection, sepsis, amputation, and mortality. Recent research suggests that vascul...

Inflammatory and Angiogenic Abnormalities in Diabetic Wound Healing: Role of Neuropeptides and Therapeutic Perspectives

Diabetic foot ulceration (DFU) is one of the most costly and debilitating complications of diabetes and is the leading cause of non-traumatic amputations, affecting 15% of the diabetic population. Impaired wound healing in diabetic patients without large-vessel disease has been attributed to microvascular dysfunction, neuropathy, and abnormal cellular and inflammatory responses. These abnormali...

The influence of polymorbidity, revascularization, and wound therapy on the healing of arterial ulceration

OBJECTIVE An ulcer categorized as Fontaine's stage IV represents a chronic wound, risk factor of arteriosclerosis, and co-morbidities which disturb wound healing. Our objective was to analyze wound healing and to assess potential factors affecting the healing process. METHODS 199 patients were included in this 5-year study. The significance levels were determined by chi-squared and log-rank t...

The Role of Nitric Oxide Donors in Wound Healing in Diabetes Mellitus

Introduction: Diabetic foot ulcer is a serious complication of diabetes mellitus; it consists of lesions in the deep tissues associated with neurological disorders and peripheral vascular disease in the lower limbs. Delayed wound healing in diabetes leads to long-term hospitalization and even amputation of distal organs. Diabetes mellitus is associated with decreased nitric oxide bioavailabilit...
